The text (and some of the arrows) I drew with pen and paper, then scanned in. The rest (boxes, lines) was done with Inkscape, which is available for Linux, Mac OS and Windows. Inkscape has this cool trace bitmap command, so you can import a scanned bitmap, turn it into a vector, clean it up, and do whatever else you need (resize, rotate, add extra elements, etc.) – you generally then save that as an SVG, and also export a PNG at some suitable resolution for putting on the web.
